  • 1915(i) Letter of Approval
  • 1915(i) Assessment
  • Psychological evaluation by a physician with diagnostic codes
  • Guardianship documentation

Innovations Waiver

  • Most recent Individual Support Plan (ISP)
  • Risk/Support Needs Assessment (RSNA)
  • Health Risk Assessment (HRA)
  • Guardianship documentation
  • Psychological (with diagnostic codes) and SIS evaluation
